An access system can be defined as any entity whose purpose is to provide access through a height. They are normally classified based on inclination to the horizontal, as ladders (vertical and inclined), stairways, and walkways. They can be formal or informal. Mortality and morbidity statistics indicate falls from access systems are highly represented in producing damage to people (in the workplace, home and public places). In order to gain insight into these occurrences, a large number of permanent disabling and fatal access system incidents were reviewed. In an overwhelming majority of cases reviewed, mismatches were found between human physical and perceptual capabilities and expectations and access system design. The study indicates the field of ergonomics (human factors) has not yet been adequately applied to the design of access systems. The presentation offers the following as a potentially different perspective to the problem; although falls of people recur with devastating regularity, perhaps it is remarkable that they don't recur more frequently, given the access systems people are required to negotiate and the nature of the highly complex human biomechanical and perceptual systems.
