This paper and the accompanying file on the CD-ROM describe the application of Six Sigma as a strategy to improve a hospital medication administration system. It is presented in conjunction with seven other papers and seven other sets of slides that together constitute the Healthcare Track for the American Congress on Quality 2003. The seven presentations review one or more aspects of DMAIC (Define, Measure, Analyze, Improve, and Control). This paper will show how each phase was applied to an actual case. The accompanying presentations are cited at the beginning of each section. NB: Before beginning - many of the examples here could have been improved with a modern information technology systems. At the time of the project that option was not available. Indeed, the IT human habit interface is proving to be a major barrier to deploying strategies such as computerized physician order entry systems. Many hospital systems have repeatedly delayed their deployment and some have temporarily stopped their deployment. The purpose of the example is to show the tools of Six Sigma can be used.
