This paper reviews 31 significant tailing incidents that have occurred during the past thirty years. Incidents are reviewed from the standpoint of design, seismic, hydrologic, and operational risk. It was found that operational risks represent the greatest risk to tailings facilities, but are seldom addressed. Recommendations are presented on methods of minimizing operational risk.
