Failure Modes And Effects Analysis (FMEA), a recognized qualitative method of risk analysis, has recently been used by Ontario Power Generation as a part of periodic dam safety reviews in conjunction with standards based reviews. This paper presents a summary of the application of the FMEA to a pilot project at the Sir Adam Beck Development Pump Generation Station dyke. It includes an overview of the FMEA outlining the purpose, process, participants and products. The benefits of the FMEA technique as an enhancement of the standards based review are demonstrated using specific examples from the FMEA of the PGS Dyke.
