Intersection operational issues leading to unsafe conditions is one of the focus areas in the United States as identified by the Federal Highway Administration. Arizona is one of the focus states currently working on strategies and safety enhancements to reduce and/or prevent intersection fatalities and injuries at intersections. The Arizona Road Safety Assessment (RSA) Program has been established and operating under the federal safety grants. During the last 2 years, there were a number of RSAs performed on the State, local as well as tribal roadway system in which more than twenty five at-grade intersections were critically reviewed and assessed. Some of the traffic design elements that appeared to be the major issues contributing to intersection-related traffic crashes are: sight distance and visibility, signing and marking, traffic signal operations, street lighting, access control, pedestrian/bicyclist facilities, ADA compliance, intersection geometry, and others. All of these factors directly or indirectly impact the intersection LOS. This paper discusses several operational issues identified at intersections and corresponding recommendations made through RSAs. Even though the examples will be cited from Arizona, most of the issues are typical throughout the country.
