In Sepetember 1996,an ethylene decomposition occured in a reactor at Lyondell Polymers' Victoria High Density Polyethylene (HDPE) facility.The plant uses a solution process for HDPE manufacture,and,prior to this incident,believed that the reaction area was unlikely to experience an ethylene decomposition due to hte large amount of solvent diluting the ethylene used in the process.The results of the investigation showed how this unrecognized possibility did actually occur,and gives insight into how a history of good process experience and changes within normal operating parameters can mask subtale process changes and result in a process safety incident.
