The paper describes an incident which occurred in a furnace operating at over 400℃. This caused failure and release of hot oil at a thermowell to body boss-joint while attempts were being made to close a leak. This case aims to define and investigate likely factors leading to failure. Whilst normal operating conditions lead to minimal loadings, attempts to plug the leak on-line, abnormal lateral loadings caused the failure.
