Recognizing the growing need for a unified problem solving methodology for resolving operations problems, as well as a unified system for archiving data for immediate lessons learned access within any company, ARES Corporation studied, tested and evaluated available technologies for performing root cause analysis (RCA). Critical data-quality elements of verifiable accuracy, consistency, objectivity, reliability and relevancy were the major components considered.rnThis paper details the criteria that were developed to evaluate RCA methodology and lessons learned concepts. It also illustrates how meeting those criteria can improve and unify problem-solving capabilities. In the study, the criterion for data quality was ranked highest in importance. Critical elements of data accuracy and its relevance to effective corrective and preventive maintenance were examined as well. Relevancy of the criteria to the aerospace industry, as they impact processes, goals, and strategies for operations reliability, is discussed.rnThis paper also describes, as a case study, how ARES, working with a large, aerospace company, identified a RCA methodology and tool and utilized the tool as the company's baselined RCA tool. All aspects of the case study are investigated including:rn1.Factors critical for the selection of the methodology and toolrn2.Various applications for which the methodology and tool have been utilizedrn3.Hurdles and challenges to determining the appropriate tool and how to overcome themrn4.Specific applications of the tool to the problems of the companyrn5.Management and personnel reactions to the tool and methodologyrn6.Other lessons learnedrnAdditionally, this paper explains how software technology can enhance and standardize an existing RCA process within any organization. The methodology and lessons learned design capabilities of the new era in RCA software technology and lessonsrnlearned systems are explained and illustrated with the REASON? software tool. REASON? illustrates how to mitigate systemic problems, thus producing cost-effective solutions.
