A redesign and retubing of the primary reformer was included in the revamp of the Kujang Fertilizer ammonia Plant. The new tubes had several problems, including a rupture in the second year of operation and embrittlement cracking of unstress relieved welds in the sixth year of operation, both of which led to plant shutdowns. These failures, and measures to prevent future failures, are explored.
