The first Advanced Extremely High Frequency vehicle, AEHF-1, encountered an anomaly in 2010 upon use of the bipropellant engine during transfer maneuver operations which was eventually overcome. Corrective actions were implemented and five "lessons learned" were also developed as a result of the problem. Due to proprietary and restricted information controls at that time, these lessons were not released to the wider community although systems engineering reviews were conducted on other government programs to avoid the same issue. While the corrective actions have prevented a recurrence of the specific issue, problems have been encountered on other programs that relate back to the same five lessons that had been generated. This paper discusses those five lessons within their original context while alluding to other instances of the same problematic aspects.
