Abstract: Arthroscopic synovectomy, which is limited today to thelarge joints, is an important early treatment ofrheumatoid arthritis (RA). Photodynamic therapy (PDT)is potentially to be a less invasive method of removingthe synovial membrane. Therefore, in a rabbit model ofRA, the accumulation of the photosensitizerProtoporphyrin IX (PPIX) after intra-articular andsystemic application of ALA into arthritic rabbit kneejoints was studied in skin, patella, synovial tissue,and meniscus by fluorescence microscopy. PPIXfluorescence was measured in biopsies taken atdifferent times after application of neutral and acidALA solutions. Significant PPIX fluorescence wasobserved in the synovial membrane and skin 2 and 4hours after application. Using intra-articularapplication, ALA solutions prepared with pH 5.5 were atleast as efficient as neutral solutions in sensitizingthe synovial membrane. Skin also showed PPIX within 4hours after application. After 24 hours, a marginalPPIX fluorescence was detected in these tissues. On theother hand, in cartilage and meniscus significant PPIXaccumulation was still observed 24 hours after ALAinjection. Systemic application of ALA also showed agood accumulation of PPIX. Further experiments areneeded to show whether accumulation of thephotosensitizer and tissue selectivity are sufficientfor a successful treatment of rheumatoid synovitis. !8
