Abstract: We conducted a study to determine if preserved humanamniotic membrane can reduce corneal haze induced byexcimer laser photoablation. Excimer photoablation wasperformed bilaterally on 40 New Zealand white rabbitswith a 6 mm ablation zone and 120 micrometer depth(PTK) using the VISX Star. One eye was randomly coveredwith a preserved human amniotic membrane and securedusing four interrupted 10 - 0 nylon sutures; the othereye served as control. The amniotic membranes wereremoved at one week, and the corneal haze was gradedwith a slit-lamp biomicroscopy by three masked cornealspecialists (WC, KH and RF) biweekly for the ensuing 12weeks. Histology and in situ TUNEL staining (forfragmented DNA as an index for apoptosis) was performedat days 1, 3 and 7 and at 12 weeks. One week afterexcimer photoablation, the amniotic membrane-coveredcorneas showed more anterior stromal edema, whichresolved at the second week. A consistent grading oforganized reticular corneal haze was noted among thethree masked observers. Such corneal haze peaked at theseventh week in both groups. The amnioticmembrane-covered group showed statistically significantless corneal haze (0.50 plus or minus 0.15) than thecontrol groups (1.25 plus or minus 0.35) (p less than0.001). The amniotic membrane-covered corneas had lessinflammatory response at days 1 and 3, showing nearlynil DNA fragmentation on keratocytes on the ablatedanterior stromal and less stromal fibroblastactivation. There is less altered epithelial cellmorphology and less epithelial hyperplasia at 1 week inthese amniotic membrane-treated eyes. We concluded fromthis study that amniotic membrane matrix is effectivein reducing corneal haze induced by excimerphotoablation in rabbits and may have clinicalapplications. !44
