While the expression of pro-inflammatory cytokines BMP-4 and TGF-βl, ECM remodeling markers cathepsin L, and MMP-2 was upregulated under elevated FSS magnitude (at normal frequency) or elevated FSS frequency (at normal magnitude), it was either maintained or even downregulated in response to combined alterations in both FSS magnitude and frequency. Therefore, when combined, FSS magnitude and FSS frequency appear to have an attenuating effect on each other. This study demonstrates the sensitivity of valve leaflets to both WSS magnitude and pulsatility and the ability of supra-physiologic WSS magnitude or pulsatility to trigger events involved in early CAVD pathogenesis, which confirms our initial hypothesis that FSS alterations are key regulators of early CAVD progression.
