摘要

The neurosurgical methods to be considered are 1(deg)) modulative. by using implanted drug delivery systems or electrostimulation, 2(deg)) ablative, by making selective therapeutic lesions in well-defined and identified targets proved to sustain pain mechanisms.According to the definition from the International Association for the Study of Pain (I.A.S.P.), Neuropathic Pain is the one initiated or caused by a primary lesion or dysfunction of the nervous system. Neuropathic pain is generally considered to be refractory to opioids as well as to ordinary analgesic treatments. Some relief can be obtained with: anticonvulsants (carbamazepine, clonazepam, valproate, gabapentin,..) tricyclic antidepressants (amitriptyline, imipramine, desipramine, clomipramine). When all medical therapies have failed, Functional Neurosurgery can be indicated in well-selected cases. Before surgery is indicated, and for the choice of the most appropriate procedure to a particular patient affected with neuropathic pain, the anatomical and physiological mechanisms at the origin of the pain have to be analyzed carefully.
