Susquehanna has a history of cyclic training performance which has caused probationary periods as recent as 2010. This trend was recognized by the station in 2016 and this prompted comprehensive changes which have served as the foundation of sustained improvement. Susquehanna was placed in the Special Focus program in 2014 due to three consecutive INPO 3 ratings. Training was consistently used to improve performance throughout the station recovery plan; however, the health of the training programs was not a focus. When the station successfully emerged from the Special Focus program in early 2016, senior leaders directed a comprehensive effort to address a decline in performance, particularly in Operations Training. Leadership changes focused on transparency when identifying, communicating and resolving issues. This led to the recognition that a more thorough evaluation was necessary, and a root cause analysis (RCA) was chartered to explore the cyclic trend in operator program performance from 2008 through 2016. Corrective actions from this analysis focused on organizational behaviors, especially leadership and teamwork.
