In the Spring of 2001, vibration on one of the Reactor Coolant Pumps in Unit 3 of the Palo Verde Nuclear Generating Station began to show indications of a shaft crack. The Station had experienced another shaft crack in Unit 1 in 1996 and monitoring and analysis proceeded on the assumption that these new indications would follow the same pattern as the previous failure. After monitoring for some time it became clear that this new problem wasn't following the same pattern. The rate of increase in vibration amplitude and phase, and eventually other 'non-crack like' symptoms indicated that the problem, although indicating a crack, was very different from the 1996 event. When the pump was eventually disassembled, an axial crack was found in the journal bearing shaft protection sleeve.
