A premature failure of an Inter module Return FanrnAssembly (IRFA) within the Columbus module of thernInternational Space Station (ISS) has been investigatedrnto identify the root cause and the counteractions to berntaken. The dismounted parts and the collected debrisrnhave been analysed using optical microscopy, ScanningrnElecron Microscopy (SEM), Energy Dispersivernspectroscopy (EDS) and dedicated dimensionalrnmeasurements have been performed.rnThe inspection indicated an anomalous wear of onernonly of the two support bearings and in particular arnstrong interaction between the cage and the lands of thernouter ring with consequent friction and wear, while thernother parts of both the bearings were in reasonablernconditions.rnThe analyses indicated a misalignment of one bearingrnthis being the origin of the non-nominal loadrnconditions that led to the premature wear of thernbearing. The inspections conducted allowed thernidentification of the corrective actions to bernimplemented in order to minimize the likelihood ofrnoccurrence of the failure.
