摘要

HIV TIDES---tailored interventions for management of depressive symptoms in persons living with HIV/AIDS (PLWHA) was designed to provide tailored, computer-based education on key elements and self-care strategies for managing depressive symptoms based on an initial assessment of depressive symptoms. Following preliminary work focused in the conceptual foundations and tailoring algorithm, five study phases were completed: (1) needs assessment, (2) HIV TIDES prototype development, (3) heuristic evaluation, (4) usability testing, and (5) pilot study. A needs assessment was conducted through a focus group with HIV field workers on the topic of mental health needs, including depressive symptoms, in PLWHA. Participants reported that PLWHA have unmet mental health information needs. They also reported that personal factors of the recipient, accessibility issues, and the design of the information intervention might affect the acceptance of information intervention. The tailoring framework of HIV TIDES was revised based on the results from the needs assessment. The decision rules and messages were developed based upon the tailoring framework. The prototype was created with Dreamweaver MX2004 using HTML, JAVA script, and PHP computer language and supported by the MySQL database. The iteration of the prototype was based on the results of the heuristic evaluation and the end-user usability test. The final version of HIV TIDES consisted of 73 messages with an average readability level of 6.0 grade. The pilot study assessed the acceptance of HIV TIDES with thirty-two PLWHA recruited from HIV/AIDS clinics. The majority of the sample was African American (68.8%), male (65.6%), with high school or lower education (68.7%), and in their 40s (62.5%). Participants spent an average of 10.4 minutes (SD = 5.6) using HIV TIDES. The average score of system acceptance was 102.3 (SD=10.6) out of a possible 110. Race/ethnicity (p.05), Internet experience (p.05), level of depression (p.05), and level of self-efficacy of shared decision making with health care providers (p.01) were factors that associated with the acceptance of HIV TIDES. The results of this study provided evidence that HIV TIDES was accepted by PLWHA. A randomized controlled trial is needed to assess the efficacy of HIV TIDES in a clinical setting.
