Effect of obesity on motor functional outcome of rehabilitating traumatic brain injury patients.

Obese patients have been marginalized in the clinical setting due to multiple complications. This has been proven to be true in rehabilitation. There has not been studies on obesity's effect on patients that are rehabilitating from traumatic brain injury (TBI). This study was designed to test obesity's effect on functional motor outcome of patients rehabilitating after a TBI. Subjects recruited into the North Texas Traumatic Brain Injury Model Systems (TBIMS) were used to obtain necessary height and weight. Five hundred seventy three subjects recruited from January 2010 to September 2013 were used. Patients who are 16 years of age or older and have an abnormal functional independence measure (FIM) motor score at admission to inpatient rehabilitation. Excluded patients were those in inpatient rehabilitation exceeding 90 days and whose time from injury to arrival to rehabilitation were 30 days or more. Subjects with concomitant non-traumatic neurologic deficits or who died during their stay were excluded. A t-test was used to compare FIM motor in both groups, and a linear regression model was fit to the data using motor FIM at discharge as the dependent variable. The recycled prediction method was used to determine the marginal impact of obesity. Of the 431 patients that had extractable data, 54 were obese. The percentage of FIM motor change was 70.2 for the non-obese and 100.2 for obese (p-value = .0639). The FIM motor at discharge for obese and non-obese was 63.1+/- 12.5 and 62.5+/-10.1, respectively. Both groups were matched for all demographic data except marital status. The proposed practicum project was designed to test the hypothesis that obesity has a negative effect on functional outcome of patients that are rehabilitating after suffering TBI, resulting in smaller gains in FIM scores than their lean counterparts from admission to discharge. It was found that, this was not the case and obese patients make just as much functional gains as non-obese patients.
