The recent repeal of the ban on federal funding for syringe exchange programs (SEPs) for the prevention of blood-borne diseases, primarily HIV, for injecting drug users (IDU) presents an opportune time to understand and recommend the most effective practices for operation and to highlight future direction. In order to assess the issues, a strengths, weaknesses, opportunities, and threats analysis (SWOT), was used to organize information gathered from the literature and perspectives gathered through interviews with current and former employees of the programs in New Haven and Hartford, Connecticut. This methodology was used to construct SWOTs to identify the critical factors that contribute to an ideal program as well as case studies of both the New Haven and Hartford sites. The strategic recommendations proposed in this thesis summarize the identified opportunities for each of the SEPs and are used to further communicate about a program's possibilities for new efforts and to determine the best decisions and actions necessary in a changing environment with identified external threats.;All three SWOT analyses highlighted the need and opportunity for a comprehensive package of services to increase client access to healthcare services. Other significant opportunities include a client-centered based approach that is culturally appropriate and includes training of harm reduction staff for tailored services to clients, as well as the creation of an electronic system that includes a client database and system of referrals that can be used for improved monitoring and evaluation, and securing community and law-enforcement support through advocacy efforts. Lastly, the repeal of the ban on federal funding and favorable political environment, present a tremendous opportunity to secure federal dollars and prioritize them for services that will contribute the greatest significant public health impact for IDUs.
