Background. C57BL/KsJ-leprdb/lep db (db/db) mice exhibit the essential characteristics of type 2 diabetes, with hyperinsulinemia, insulin resistance, and hyperglycemia. It has previously been shown with ex vivo perfusions that hearts from diabetic db/db mice exhibit a cardiomyopathy, with reduced contractile function and altered metabolism. To evaluate whether there is evidence of cardiac dysfunction in vivo, echocardiographic parameters were assessed in intact diabetic db/db mice at both an early (6 weeks of age) and advanced (12 weeks of age) stage of diabetes. Echocardiography provides a non-invasive method to assess both systolic and diastolic function.; Methods. Echocardiograms were obtained in 6 and 12 week conscious and anaesthetized db/db mice, and compared to their 6 and 12 week conscious and anaesthetized control db/+ non-diabetic lean littermates.; Results. In conscious 12 week db/db mice echocardiograms, fractional shortening and circumferential shortening were significantly decreased (59.5 ± 2.3 versus 42.8 ± 2.1% and 11.8 ± 0.4 versus 8.3 ± 0.5) indicating systolic dysfunction. The anaesthetized 12 week db/db mice had significantly increased peak A velocity (0.35 ± 0.03 versus 0.45 ± 0.03 m/sec), and a decreased E/A ratio (3.56 ± 0.29 versus 2.40 ± 0.20) and deceleration time (0.04 ± 0.002 versus 0.05 ± 0.002 sec), evidence of diastolic dysfunction. These changes were not yet detectable in the 6 week db/db mice.; Summary. The decreased in vivo systolic and diastolic function in the 12 week db/db mice confirm the results of cardiac dysfunction in the ex vivo perfusions. Important new insight into the possible mechanisms responsible for cardiac dysfunction in this experimental model of Type 2 diabetes mellitus are given.
