As part of the Iowa Farm Family Health and Hazard Surveillance Project (IFFHHSP), a multi-year health and safety study focused on the agricultural community in the State of Iowa, a rapid health and safety assessment tool---Site Rank System---was developed and applied to 121 Iowa farm operations visited during a one year, cross-sectional, on-farm industrial hygiene (IH) phase (July 1995 to June 1996). A single investigator was utilized to conduct all on-farm IH visits, that were randomly selected from a stratified, two-stage cluster sample of Iowa farm operations. The Site Rank System was based on the average of scores assigned to four different farm characteristics (farm operator attitude, farm operator practices, facilities and equipment). This system was assigned consistently, but it showed little correlation to self-reported risk factors. However, self-reported injuries occurring to the principal farm operators of the farm operations visited correlated highly with low Site Rank System scores. Further refinement and validation of this approach is warranted.As part of the IFFHHSP, a portable "pen-based" computer system was developed to accurately record the frequency, duration and sequence of farm operator on-farm tasks and activities. It incorporated "pen-based" or "grid" computer software and portable hardware, along with a highly specialized computer "run-time" program---Hazard Evaluation and Activity Recording System (HEARS). The development and testing utilized to create the HEARS run-time program, along with its logic, structure and shortcomings are presented.The HEARS run-time program was successfully applied to a sub-sample (45 farm operations) of the 121 Iowa farm operations visited during the on-farm IH phase of the IFFHHSP. The different types and duration of on-farm tasks and activities observed, along with the results from an analysis of equipment-related "near-miss" hazard scenarios (NMHS) are presented. Overall, the results from the analysis of the data gathered with the HEARS run-time program represent new information relative to the agricultural setting. Further, it is concluded that the advantages this type of technology provides clearly outweighed any disadvantages.
