Red blood cell lactate transport in sickle cell disease and sickle cell trait.

The purposes of this study were to: (1) determine the rate and mechanisms of lactate transport in red blood cells (RBC) of persons with (a) sickle cell disease and (b) sickle cell trait; and (2) compare the mechanisms of lactate transport in RBC of persons with sickle cell disease and sickle cell trait to the mechanisms of lactate transport in a group of subjects not possessing sickle hemoglobin. IRB forms were submitted and approved by both the Auburn University and University of Alabama in Birmingham. Blood samples were obtained from 30 African American subjects. The influx of lactate into RBC was measured by a radioactive tracer technique using 14C-lactate at six (2, 5, 10, 15, 25, and 40 mM) lactate concentrations ([La-- ]). PCMBS (1 mM) was used to block the MCT pathway, and determine the percent contribution of this pathway to total lactate influx in RBC from the above three groups. In general, total lactate influx into RBC from the HbSS group was significantly faster than influx into RBC from either the HbAA group or the HbAS group. Lactate influx via the MCT pathway into RBC from the HbSS group was significantly faster than lactate influx via this pathway into the RBC from either the HbAA group or the HbAS group at all lactate concentrations, except 2 mM. RBC from the HbSS group showed a greater percent of lactate influx through the MCT than did RBC from the HbAA group at 2, 15, 25 and 40 mM [La --], and greater than RBC from the HbAS group at 15, 25 and 40 mM [La--]. Both the Km (42 +/- 8 mM) and Vmax (4.7 +/- 0.6 mumoles ml--1 min--1) for RBC from the HbSS group were significantly greater than the Km (27 +/- 12 mM) and Vmax (2.0 +/- 0.5 mumoles ml--1 min--1) values for RBC from the HbAA group. The Vmax for the RBC from the HbSS group was also significantly greater than the Vmax for the RBC from the HbAS (2.9 +/- 1.5 mumoles ml--1 min --1) group. We proposed that stimuli related to metabolic alterations due to chronic anemia might serve to increase expression and/or activity of the MCT carrier on the RBC of persons with HbSS.
