Early Psychiatric Intervention in Children and Adolescents with Psychosis.

Mental illness is increasing among children and adolescents, with 1 out of every 5 children being diagnosed with a mental disorder. The onset of psychosis typically occurs in childhood and adolescence and, in addition to being particularly refractory to most treatments, is associated with high costs of hospitalization and behavioral health care, high school suspensions, poor academic performance, and limited career opportunities. The purpose of this project was to plan for adaption and implementation of the Early Psychotic Prevention Intervention Center Program at a Maryland mental health clinic modeled on the experience, guidelines, and standards of the Australian Early Psychotic Prevention Center Program and the Ministry of Health Services Province of British Columbia. The IOWA model for evidence-based practice to promote quality care was used to facilitate the change within the clinic. The practical and theoretical support for the project included McGorry's clinical staging for psychosis treatment, Lewin's theoretic model for change and force field analysis, Juran's model for quality planning, and the functionalism model for implementing social change. The detailed plans for implementation include timelines, staffing recommendations, licensure and credentialing considerations, financial planning, and marketing that will assist other clinics as they move to early detection and treatment of psychosis in children and adolescents. The BASIS-24 and the sign test will be used to evaluate the ongoing outcomes of the clients and the project. Successful treatment of young persons with psychosis will result in important social change related to the mental illness. Early intervention can be effective in improving the quality of life for patients and family members and can reduce the high costs of care that result when the symptoms of psychosis go untreated.
