Organizational readiness factors: are we ready to adopt and implement clincial best practices in hospitals?

Organizations pursuing strategies to achieve excellence often look to adopt innovative best practices. This study investigated the adoption of best practices, applying Rogers' diffusion of innovations theory and its implementation stage and the organizational readiness factors that affect implementation. The literature review revealed that the implementation change process is influenced by several organizational readiness factors: planning, budgeted resources, organizational learning and communication processes. A representative sample of perinatal units in the United States was surveyed to explore the relationship between these factors and the adoption and implementation of perinatal clinical best practices. Overall, 88% of respondents adopted and implemented an average of 5.7 best practices; 68% did so within 6-12 months. While all organizational readiness factors had a positive correlation, the strength of each relationship was moderate, except for organizational learning which showed a significant relationship. Significance tests, correlations and the prediction of the number of clinical practices adopted and implemented as a result of the organizational learning factor reflected a th-value of .000, an R2 value of .304, and an f- value of 49.8. A beta weight of .399 and a th-value of .002 further support organizational learning as a predictor of successful implementation of best practices. This suggests that readiness characteristics transcend beyond the traditional classroom. Managers need to understand organizational learning as the recognition of an acquired learning need through diverse activities such as reflection, feedback from observer to learner, and open forums for idea generation, each designed to embed the expectation of learning into daily routines The level of importance associated with the ability to maintain a best practice was significant for communication, with a th-value of .000 and a structure coefficient of .930. This factor was deemed important in the initial implementation stage and evolved as more important in the maintenance stage. The organization that has successfully embraced an implementation management approach has shifted a manager's mindset from simply deciding to implement a best practice to understanding the importance of certain organizational readiness factors in stabilizing the implementation of a best practice.
