目的 比较TEP和无张力修补腹股沟斜疝方法的手术疗效.方法 将78例患者按住院手术治疗的先后顺序随机分为TEP组33例,无张力修补组45例.比较两组患者手术时间、治疗费用、住院天数以及术后并发症等指标,以评价TEP和无张力修补腹股沟斜疝的疗效.结果 TEP组随访时间平均(16±1.5)个月,无张力修补组(24±1.8)个月,完成随访率为97.43%.无张力修补组2例1年半后复发(4.44%),TEP组至今复发1例(3.03%),手术时间、平均住院天数、治疗费用、疝复发率、术后发热和尿潴留发生率等均无明显差异(P>0.05);术后疼痛、阴囊水肿、切口感染和缺血性睾丸炎等近、远期并发症TEP组明显少于无张力修补组,两组间差异有显著意义(P<0.05).结论 TEP修补腹股沟疝可明显减少各种并发症和术后不适的发生率,疗效肯定,值得临床推广应用.%Objective To compare the surgical efficacy between TEP and tension -bee repair of inguinal hernia. Methods 78 patients were randomly assigned into TEP group of 33 patients,and tension-free repair group of 45 patients. The two groups were compared in operative time,cost of treatment,length of hospital stay ,and postoperative complications to evaluate the efficacy . Results The average follow-up time of the TEP group was (16 ±1.5) months, tension-free repair group ( 24 ± 1. 8 ) months. The follow-up completion rate was 97.43%. As for tension-free repair group two cases recurred a year and a half later (4. 44% ) , which TEP group had relapsed patient (3.03% ). There were no significant differences in operation time , average length of stay, the cost of treatment, hernia recurrence rate, postoperative fever and urinary retention incidence (P >0. 05) ;postoperative pain ,scrotal edema,wound infection and ischemic orchitis recent long-term complications in TEP group were significantly less than those in the tension -bee repair group. The difference between the two groups was significantly significant (P <0.05). Conclusion TEP repair of inguinal hernia can significantly reduce the incidence of various complications and postoperative discomfort , which is effective and worthy of clinical application .
