出院指导加随访对慢性心力衰竭患者出院30天内再次入院的影响

摘要

目的：观察出院指导加随访对慢性心力衰竭（心衰）患者出院后30天内再入院的影响。方法对2015年1～12月心衰患者出院后给予出院指导加随访86例，作为随访组；回顾分析2013年11月～2014年10月心衰出院患者未规范随访80例，作为对照组；观察两组患者30天内再入院情况。结果出院后30天内再次住院情况随访组为13例，再入院率15.5%。对照组再次住院为24例，再入院率30%。再入院率两组比较，P＜0.05，差异有统计学意义。结论慢性心衰患者出院后给予出院指导加随访可明显减少出院后30天内的再住院率。%ObjectiveTo observe the effect of discharge instructions combination follow-up on 30 days readmission following hospitalization for heart failure (HF).Methods 86 patients with chronic heart failure were divided into the experimental group after their discharge from January to December 2015 and acceped discharge instructions and follow-up, 80 patients after discharge from November 2013 to October 2014 were in the control groups without discharge instructions and follow-up. Two groups of patients were observed in 30 days of re admission. ResultsWithin 30 days after discharge, the patients were followed up for 13 cases, the rate of re admission was 15.5%.In the control group, 24 patients were hospitalized again, and the rate of admission to hospital was 30%. Readmission rate of two groups,P<0.05, the difference was statistically signiifcant.ConclusionPatients with chronic heart failure after discharge guidance and follow-up can signiifcantly reduce the rate of re hospitalization within 30 days after discharge.
