目的：分析并评价慢病管理对社区高血压患者的治疗效果。方法从我社区门诊部2010年6月～2015年5月间接收的高血压患者中随机选择86例，按照干预方法进行分组，实施了慢病管理治疗的51例患者纳入观察组，仅实施常规健康指导的35例患者纳入对照组。调查并比较两组患者的健康知识、生活质量测评结果，并详细分析两组的血压控制情况。结果观察组的健康知识测评优良率（49/51）要大于对照组（24/35），生活质量评分高于对照组，组间对比具有统计学意义（P 均＜0.05）。观察组的血压控制率（90.2%）高于对照组（62.9%），对比差异有统计学意义（P ＜0.05）。结论慢病管理能够促使患者养成合理的用药习惯、良好的生活习惯，对于改善社区高血压患者的治疗效果有重要意义。%Objective To analyze and evaluate the treatment effect of chronic disease management in community hypertension patients. Methods 86 patients with hypertension received from June 2010 to May 2015, 51 patients with chronic disease management were included in the observation group, and 35 patients with conventional health guidance were included in the control group. The health knowledge and quality of life of the two groups were investigated and compared, and the blood pressure control of the two groups was analyzed in detail.
