Objective To study the clinical effect of the different ways of surgery for glaucoma with cataract.Methods Selected 84 cases glaucoma with cataract patients randomly were divided into three groups. The patients were adopted two-stage surgical method in group A. The patients were treated by simple cataract ultrasonic emulsification combined with intraocular lens implantation surgery in group B and the patients were used ultrasonic emulsification cataract gettering artificial lens implantation and trabeculectomy in group C. The preoperative and postoperative vision changes and intraocular pressure changes of three groups of patients were recorded and patients with complications.Results The postoperative visual acuity of three groups of patients were increased and the increase was most pronounced in patients of group C with preoperative patients with less than 4.0 of 72.5%, the eyesight of 4.0 to 4.4 of 25.0% of patients, eyesight to 4.5 to 4.6 of 2.5% of patients reduce to 12.5% lower than 4.0 patients after treatment, the eyesight of 4.0 to 4.4 in patients with reduced to 20.0%, 4.5 to 4.6 patients increased to 27.5%, 4.7 to 4.9 patients increased by 25.0%, and 20.0% patients with vision above 5.0, the postoperative intraocular pressure significantly of three groups of patients was reduced,P＜0.05, was difference had statistically significance. the comparison between three groups had no significant difference,P＞0.05, was no difference had statistically significance. Three groups of complication rates were 19.1%, 17.8% and 19.1% respectively and compared between three groups,P＜0.05, was no difference had statistically significance. Conclusion The clinical efficacy of ultrasonic emulsification cataract gettering artificial lens implantation and trabeculectomy surgery for glaucoma with cataract patients was safe and reliable, which was the best curative effect.%目的 分析研究我院收治的青光眼合并白内障患者的不同手术方式治疗临床疗效.方法 84例患者按照手术方式的不同将病例分为A、B、C三组;A组分步治疗,B组白内障超声乳化结合人工晶体植入手术,C组超声乳化白内障吸除联合人工晶状体植入联合小梁切除术,对患者的术前情况与术后情况随访记录.结果 三种手术治疗手段均可以有效提高患者的视力,治疗前后比较,P＜0.05,差异具有统计学意义,C组术前低于4.0的患者占72.5%,视力为4.0～4.4患者占25.0%,视力为4.5～4.6患者占2.5%,经过治疗后低于4.0患者降低至12.5%,而视力为4.0～4.4患者减低至20.0%,较其他两组不同, C组4.5～4.6患者增加至27.5%,4.7～4.9患者增加25.0%,并且视力高于5.0患者20.0%,增加的幅度最大,提高最为明显.三组患者的眼压在治疗后均有所降低,治疗前后比较,P＜0.05,差异具有统计学意义.A组并发症发生率25.0%;B组并发症发生率25.0%;C组并发症发生率22.5%,P＞0.05,差异不具有统计学意义.结论 采用超声乳化白内障吸除联合人工晶状体植入联合小梁切除手术治疗青光眼合并白内障患者,具有良好的临床疗效和安全性.
