目的：为了能快速判断入院压疮高危病人，并进一步给予系统评估和规范化护理措施。方法：建立压疮高风险病人预警评估信息系统，包括病人入院压疮高危初评、压疮高风险评价、系统提示压疮预防和护理措施、压疮上报等系统内容。结果：系统的建立可以帮助护士在病人入院时快速识别压疮高危病人，及时给予压疮专业评估，提高入院压疮高危病人风险评估率；护士在系统引导下给予专业的护理措施，促进护士的压疮护理措施规范化。结论：压疮高风险病人预警评估信息系统可以提高护士的压疮高风险评估率，规范化护士的压疮预防护理措施。%Objective: To estimate the admitted patients who are in high risk of pressure sore, and to assess the patients further. Methods: Development the information system of patients assessment at pressure sore high risk for early warning, including the admission high assessment, and the system prompts the prevention and nursing measures of pressure ulcer, reporting system. Results This information system can help nurses distinguishing the high risk patients in pressure sore, and guide nurses to give the patients professional nursing prevention. Conclusion: This information system can improve the assessment ratio in pressure sore high risk of admission patients, and standardize the nurses' procedure in pressure sore prevention.
