原位微波消融与肿瘤假体置换治疗骨肿瘤的三维有限元分析

         

摘要

目的:应用三维有限元分析比较原位微波消融与肿瘤假体置换二种手术方法治疗四肢近关节骨肿瘤的生物力学性能,为临床手术治疗提供理论依据.方法:选择一例膝关节骨肿瘤患者,并与临床已经应用的肿瘤假体、锁定钢板进行CT薄层扫描,获得DICOM图像数据,导入三维有限元分析软件,分别模拟使用肿瘤假体和锁定钢板二种不同的治疗方式,应用三维有限元分析法进行两种不同治疗方式术后应力变化的测算,并对所得数据进行统计学分析.结果:两组模型应力均未达到屈服应力,肿瘤假体置换的应力分布比较均匀,最大应力为56.32 Mpa,锁定钢板的最大应力为161Mpa,两种治疗方式有统计学差异(P<0.05).结论:原位微波消融和肿瘤假体置换二种治疗方法的总体生存率达到85%,假体置换组优于微波消融组,而两种治疗方法的中远期效果如何有待进一步观察和比较.%Objective: To explore the value of endoprosthetic replacement with microwave-induced hyperthermia and locking compression plates by using finite element analysis methods, and to provide experimental evidence for choice of suitable clinical. Methods: Three dimentional (3-D) finite element models of the locking compression plates and endoprosthetic replacement were constructed using simpleware software, as well as a 3-D model of femur. The finite element analysis was performed in Abaqus. The stiffness and stress distributions were calculated. Results: Stresses of all models did not exceed yield strength. The stress of locking compression plates maximum principal stress was 161Mpa, and the endoprosthetic replacement maximum principal stress of interlocking intramedullary nails was 56.32Mpa, the distribution of the corresponding mechanical there are significant differences (P<0.05). Conclusions: Endoprosthetic replacement is better than locking compression plates, endoprosthetic replacement with microwave-induced hyperthermia is a feasible method for femur treatment and its long term effect should be further observed.
