Objective To investigate the efficacy and safety of B-ultrasound guided percutaneous puncture and drainage of gallbladder for acute calculous cholecystitis during middle and late pregnancy.Methods Forty patients with acute calculous cholecystitis during middle and late pregnancy underwent B-ultrasound guided percutaneous puncture and drainage of gallbladder were analyzed retrospectively.Clinical efficacy and untoward event incidence were observed to evaluate the efficacy and safety.Results All forty patients successfully underwent B-ultrasound guided percutaneous puncture and drainage of gallbladder.After treatment,the symptoms including abdominal pain and fever of the patients were significantly alleviated.Complications such as bleeding and biliary fistula were not observed.Laboratory parameters such as white blood cell count and total bilirubin were improved.All patients underwent routine tube removal or went through delivery with the tube and underwent tube removal when receiving elective laproscopic cholecystectomy.Twenty-two of the forty patients went through natural delivery and the other eighteen went through caesarean delivery,and all the newborns were normal.Conclusion The efficacy and safety of B-ultrasound guided percutaneous puncture and drainage of gallbladder for acute calculous cholecystitis during middle and late pregnancy were decent and should be considered as an appropriate therapeutic agent in clinical practice.%目的 探讨B超引导下经皮胆囊穿刺置管引流术治疗妊娠中晚期急性结石性胆囊炎的疗效,为妊娠中晚期急性结石性胆囊炎的治疗提供指导.方法 回顾性分析本院收治的40例妊娠中晚期急性结石性胆囊炎患者经B超引导下经皮胆囊穿刺置管引流术治疗后的临床资料,分析其临床疗效及并发症情况.结果 40例患者均成功实施B超引导下经皮胆囊穿刺置管引流术治疗,治疗后腹痛、发热等症状和体征均明显改善,无出血、胆瘘等并发症,白细胞计数、总胆红素等实验室指标较治疗前均明显好转.所有患者均常规拔管或带管分娩后择期行胆囊切除术时拔管.40例患者22例自然分娩,18例剖宫产,新生儿均正常.结论 B超引导下经皮胆囊穿刺置管引流术治疗妊娠中晚期急性结石性胆囊炎疗效显著,安全性较好,适合临床推广.
