目的 分析双微导管技术治疗颅内大型动脉瘤的近远期效果.方法 选取河北省邯郸市第一医院2012年8月～2014年2月收治的80例颅内大型动脉瘤患者,依据随机数字表法分为两组,观察组采用双微导管技术治疗,对照组采用支架辅助技术治疗,比较两组患者近远期疗效.结果 所有患者均顺利实施介入手术,术后即刻造影显示,两组患者填塞密度和栓塞程度比较,差异无统计学意义(P＞0.05).观察组血栓形成、血管痉挛发生率显著低于对照组(P＜0.05).观察组手术前后活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)比较,差异无统计学意义(P＞0.05).对照组手术前后APTT、PT比较,差异有统计学意义(P＜0.05).随访期间,未出现再出血和神经功能缺损症状,无死亡病例,观察组格拉斯哥预后评分显著高于对照组(P＜0.05);观察组复发率显著低于对照组(P＜0.05).结论 双微导管技术治疗颅内大型动脉瘤患者近远期疗效确切,创伤小,并发症少,复发率低,治疗更为安全、有效.%Objective To analyze short and long term efficacy of double microcatheter technology in the treatment of intracranial large aneurysms.Methods 80 patients with intracranial large aneurysms in the First Hospital of Handan City,Hebei Province from August 2012 to February 2014 were selected and divided into two groups according to the random number table method.Double microcatheter technology was used in observation group,stent assistive technolo gy was applied in control group.Short and long term efficacy between two groups were compared.Results All patients were involved in the successful implementation of the surgery.Immediate postoperative angiography showed,packing density and embolism between two groups were compared,with no statistical difference (P ＞ 0.05).The incidence rate of thrombosis and vasospasm in observation group were lower than those in control group (P ＜ 0.05).Activated partial thromboplastin time (APTT),prothrombin time (PT) in observation group before and after operation were compared,with no statistical difference (P ＞ 0.05).APTT,PT in control group before and after operation were compared,with statistical difference (P ＜ 0.05).Follow-up period,there were no rebleeding and neurological deficits,no death case.Glasgow prognostic score in observation group was significantly higher than that in control group (P ＜ 0.05).The recurrence rate of observation group was significantly lower than that of control group (P ＜ 0.05).Conclusion Double microcatheter technology in the treatment of intracranial large aneurysms has a certain short and long term efficacy,with less inva sive,fewer complication,lower recurrence rate,which is safer and more effective.
