目的 观察中医辨证治疗+针灸对带状疱疹神经痛临床症状的改善效果及安全性.方法 选取2014年2月～2016年10月中国中医科学院望京医院收治的带状疱疹神经痛患者98例作为研究对象,并采用随机数字表法将其随机分为观察组和对照组,每组各49例.两组患者均给予基础治疗+中医辨证治疗,观察组在此基础上联合针灸治疗.7d为1个疗程,治疗2个疗程.观察两组临床疗效,比较两组治疗前后的视觉模拟评分(VAS),记录两组止疱时间、结痂时间、疼痛开始缓解时间,以及治疗期间有无不良反应情况发生.所有患者均获得3个月的随访,观察有无神经痛复发.结果 观察组总有效率高于对照组,差异有统计学意义(P＜0.05).治疗后两组VAS均较治疗前显著降低(P＜0.05),且观察组治疗后VAS明显低于对照组,差异有统计学意义(P＜0.05).观察组止疱时间、结痂时间、疼痛开始缓解时间均明显短于对照组,差异均有统计学意义(P＜0.05).随访3个月,两组患者均无神经痛复发.两组患者治疗期间均无严重不良反应发生.结论 中医辨证治疗+针灸能有效改善带状疱疹神经痛的临床症状,且安全性高,值得临床推广应用.%Objective To observe the improvement effects of TCM syndrome differentiation + acupuncture for the clinical symptoms of herpes zoster neuralgia and its safety.Methods Ninety-eight cases with herpes zoster neuralgia admitted to Wangjing Hospital of China Academy of Chinese Medical Sciences from February 2014 to October 2016 were selected as research objects,and they were randomly divided into observation group and control group by random number table,with 49 cases in each group.Both groups were given routine treatment + TCM syndrome differentiation,on basis of which,observation group was added with acupuncture therapy.7 d was as a course,for 2 courses.The clinical effects of the two groups were observed,the visual analogue scale (VAS) of the two groups before and after treatment were compared,the over time of herpes,escharosis time,the time of pain start to relieve,and the occurrence or not of adverse reactions in the two groups were recorded.All patients were followed-up for 3 months to observe the recurrence or not of neuralgia.Results The total effective rate of observation group was higher than that of control group,the difference was statistically significant (P ＜ 0.05).The VAS scores in the two groups after treatment were sig nificantly lower than those before treatment (P ＜ 0.05),and the VAS scores in the observation group after treatment were significantly lower than those of control group,the difference was statistically significant (P ＜ 0.05).The over time of herpes,escharosis time,the time of pain start to relieve in the observation group was all shorter than that of control group,the differences were statistically significant (P ＜ 0.05).Followed-up for 3 months,both groups had no occurrence of neuralgia.There were no serious adverse reactions in the two groups during treatment.Conclusion TCM syndrome differentiation + acupuncture can improve the clinical symptoms of herpes zoster neuralgia effectively,and the safety is high,which is worthy of clinical promotion and application.
