微型钢板内固定治疗掌骨干骨折的临床疗效观察

         

摘要

目的 研究微型钢板内固定治疗掌骨干骨折的疗效.方法 选择我院2009年3月~2012年6月收治的150例因掌骨干骨折而接受手术治疗的患者为研究对象,其中90例患者(微型钢板组)采用切开复位行微型钢板及螺丝钉内固定术,60例(克氏针组)采用经手法复位或切开复位行克氏针固定术.观察两组骨折愈合时间、TAM评分及并发症发生情况.结果 微型钢板组患者的骨折愈合时间为(4.6±1.5)周,明显短于克氏针组(8.4±1.3)周(t = 3.241,P < 0.05).根据两组患者的TAM评分,微型钢板组患者的术后优良率为91.1%,明显高于克氏针组(70.0%)(χ2=7.135,P < 0.05).骨延迟愈合、骨不连在微型钢板组和克氏针组的发生率差异有统计学意义(P < 0.05),两组患者感染的发生率差异无统计学意义(P > 0.05).结论 微型钢板治疗掌骨干骨折有较为明显的疗效,较克氏针术的疗效更佳,且骨折愈合时间短,优良率高,值得临床应用.%Objective To investigate the clinical efficacy of the mini-plate fixation in the treatment of metacarpal shaft fractures. Methods 150 cases admitted to our hospital from March 2009 to June 2012 due to metacarpal shaft fracture and underwent surgery were selected as research objects, of which 90 cases (mini-plate group) were treated with mini-plate fixation and 60 cases (Kirschner wire group) were treated with manual reduction or open reduction with Kirschn-er wire fixation. The fracture healing time, TAM scoring and incidence of complication were observed in the two groups. Results The fracture healing time of the mini-plate group was (4.6 1.5) weeks, which was significantly shorter than (8.4 1.3) weeks in the Kirschner wire group (t = 3.241, P <0.05). According to TAM ratings of the two groups, good-excellent rate in the mini-plate group was 91.1%, which was significantly higher than the Kirschner wire group (70.0%) (χ2 = 7.135, P < 0.05). The incidence of delayed union and nonunion in the micro-plate group and Kirschner group had statistically significant differences (P < 0.05). The incidence of infection of the two groups had no significant difference (P > 0.05). Conclusion Compared with Kirschner surgery, mini-plate has more significant effect in the treatment of metacarpal shaft fracture surgery, and it is worthy of clinical application.
