目的 探讨不同固定方法治疗下颌骨骨折的临床效果.方法 回顾性分析我院2009年1月～2011年10月我科室收治的下颌骨骨折患者58例,根据其不同固定方法进行分组,即A组采用小钢板固定法,B组采用钛板坚强内固定术.评价治疗后口腔功能恢复情况,比较各组固定方法的效果.结果 经治疗成功率为87.93%.比较两组住院时间(t = 6.521,P ＜ 0.05)、延迟愈合率(χ2 = 4.463,P ＜ 0.05)和并发症发生率(χ2 = 4.735,P ＜ 0.05),两组差异有统计学意义.治疗1个月后两组开口度(χ2 = 6.149,P ＜ 0.05)、咬颌关系(χ2= 4.039,P ＜ 0.05)和颞颌关节功能(χ2 = 4.614,P ＜ 0.05)比较,差异有统计学意义.结论 小型钛板坚强固定术具有对患者的美学效果,口腔恢复功能及生物相容性均较小型钢板固定优良,适合下颌骨骨折固定的应用,值得临床推广使用.%Objective To explore the clinical effects of two different fixation methods for treatment fracture of mandible. Methods A retrospective analysis of 58 patients who with fracture of mandible in our hospital from January2009 to October 2011 were divided into 2 groups according to the ways of fixation method, plate fixation and micro-plates. The clinical effects and oral cavity function were compared between the two groups. Results The success rate of two groups was 87.93%. The median length of hospital stay (t = 6.521, P < 0.05), the delayed healing rates (x2 = 4.463, P < 0.05) and the incidence rate of complications (x2 = 4.735, P < 0.05) for two groups were statistical different significantly. The opening degree (x2= 6.149, P < 0.05), occlusion (x2 = 4.039, P < 0.05) and temporomandibular joint function (x2= 4.614, P < 0.05) were statistical different significantly after 1 month for operating. Conclusion The aesthetic effect, oral cavity function and biocompatibility of the miniature titanium plate fixation are better than them of plate fixation and it has great value in clinical application.
