目的：探讨单纯扩大潜式开窗减压术治疗退变性腰椎侧弯并椎管狭窄症的临床疗效。方法选取2011年7月～2014年6月福建省三明市第二医院收治的80例退变性腰椎侧弯并椎管狭窄症患者按随机数字表法分成研究组和对照组各40例。对照组采用后路减压植骨融合内固定术进行治疗；研究组采用单纯扩大潜式开窗减压术进行治疗。对比两组的手术时间、术中出血量、住院时间及治疗前、后的Cobb's角、腰背疼痛程度、Oswestry功能障碍指数、生活质量。结果研究组手术时间明显较对照组短（P＜0.01）；研究组术中出血量明显较对照组少（P＜0.01）；研究组住院时间较对照组短（P＜0.01）；研究组治疗前Cobb's角、腰背疼痛、Oswestry功能障碍指数、生活质量得分与对照组相比较无统计学意义（P＞0.05）；研究组治疗后Cobb's角大于对照组（P＜0.05）；研究组治疗后腰背疼痛、Oswestry功能障碍指数、生活质量得分均明显低于对照组（P＜0.05）。结论单纯扩大潜式开窗减压术治疗退变性腰椎侧弯并椎管狭窄症手术时间短，出血量少，患者恢复快，且术后总体疗效更好。%Objective To learn about the clinical curative effect of simple expanding latent fenestration decompression in the treatment of degenerative lumbar scoliosis combined with spinal canal stenosis. Methods 80 patients with degenerative lumbar scoliosis combined with spinal canal stenosis who were admitted to Sanming Second Hospital,Fujian from July 2011 to June 2014 were selected and randomly allocated to the research group and the control group according to random number table,with 49 in each.The control group was treated with posterior decompression bone grafting with internal fixation while the research group was treated with simple expanding latent fenestration decompression.The operation time,intraoperative blood loss, hospitalization stay,Cobb's angle before and after treatment, pain degree of waist and back,Oswestry dysfunction index and quality of life of two groups were compared.Results The operation time of the research group was significantly shorter than that of the control group (P<0.01). The intraoperative blood loss of the research group was significantly less than that of the control group (P<0.01).The hospitalization stay of the research group was shorter than that of the control group (P<0.01). Cobb,s angle, pain of waist and back,Oswestry dysfunction index and quality of life score before treatment in the research group had no significant difference from those in the control group(P>0.05).While Cobb's angle after treatment of the research group was bigger than that of the control group after treatment (P<0.05).The pain of waist and back,Oswestry dysfunction index and quality of life score after treatment were significantly lower than those of the control group after treatment (P<0.05).ConclusionSimple expanding latent fenestration decompression in the treatment of degenerative lumbar scoliosis combined with spinal canal stenosis has the advantages of shorter operation time, less blood loss,rapid recovery and better total curative effect after operation.
