小儿氨基酸治疗小儿秋季腹泻的系统评价

         

摘要

目的:系统评价小儿氨基酸治疗小儿秋季腹泻的有效性和安全性.方法:计算机检索PubMed、EMbase、Cochran图书馆、CNKI、维普、CBM等文献数据库.按Cochrane系统评价的方法评价纳入研究质量,对同质研究采用RevMan 5.0进行Meta分析,对数据不能合并者,则进行描述性的定性分析.结果:本研究共纳入5个RCT,包括673例秋季腹泻患儿.纳入研究质量均为C级.Meta分析结果显示:与常规治疗相比,小儿氨基酸+常规治疗对小儿秋季腹泻的治疗有效率更高[RR=1.32,95%CI(1.03,1.70)];在缩短止泻时间方面,小儿氨基酸+常规治疗优于常规治疗[MD=-1.66,95%CI(-2.00,-1.31)];在缩短小儿秋季腹泻治疗总疗程方面,小儿氨基酸+常规治疗优于常规治疗[MD=-2.09,95%CI(-2.39,-1.79)].1项研究报道了小儿氨基酸组和常规治疗组对血清前白蛋白的影响,两组差异有统计学意义P<0.05);1项试验报道了小儿氨基酸组和常规治疗组对血清白蛋白的影响,两组差异有统计学意义(P<0.05).所有文献均未报道两组的不良反应.结论:现有研究表明小儿氨基酸在小儿秋季腹泻的辅助治疗中能够提高治疗有效率,缩短止泻时间和治疗总疗程,改善营养丢失,也未见明显不良反应.但由于纳入的研究质量普遍偏低,上述结论尚需开展更大样本的高质量随机对照实验证实.%Objective: To assess the effect and safety of peadiatric amino compound on children autumn diarrhea. Method: All randomized controlled trails of peadiatric amino compound on children autumn diarrhea were performed. All clinical trails were searched from PubMed, EMbase, Cochran library, CNKI, VIP, CBM and other databases. Evaluating the quality of the study according to Cochrane systematic reviews, Meta-analysis was performed for the results of homogeneous studies by Cochrane Collaboration' s software RevMan 5.0, and the heterogeneous data conducted a descriptive qualitative analysis. Result: Five clinical trails including 673 patients of children autumn diarrhea met the inclusion criteria. However, the included trials were low quality and small sample. Meta analysis showed that the effect of peadiatric amino compound was better than that of routine treatment [ RR = 1.32,95% CI(1.03,1.70) ]. It was also superior to routine treatment in reducing diarrhea time [ MD = - 1.66,95% CI( - 2. 00, - 1.31 ) ] and total treatment course [ MD =-2. 09,95% CI( -2. 39, -1.79) ]. It had been reported that pediatric amino compound could influence the serum albumin and serum prealbumin, and had significant difference when compared with routine treatment ( P < 0. 05 ). No adverse effect was reported. Conclusion: Peadiatric amino compound can improve the effective rate, reduce the diarrhea time, total treatment course and the loss of children' s nutrition. No significant adverse effect is reported. Conclusive results can' t be made about the effectiveness and safety of peadiatric amino compound on children autumn diarrhea according to the limited existing trails. Larger and higher quality randomized controlled trails are needed to assess the available evidence.
