OBJECTIVE: To approach the points of clinical pharmacists participating in pharmaceutical care for severe aged patients. METHODS: Clinical pharmacists participated in treatment for a severe aged patient and put forward suggestions on nutritional support, analgesia treatment, drugs by nasal feeding and anti-infection treatment, etc. Clinical pharmacists also developed individual dosage regimen and provided medication guidance for nurse and family members of patients. RESULTS: Through pharmaceutical care, the reasonable drug therapy was provided by the clinical pharmacist and the patient got satisfactory results. CONCLUSION: Clinical pharmacists, physicians and nurses form treatment group, which is benefit to improve clinical treatment and rational drug use.%目的:探讨临床药师参与重症高龄患者综合治疗的药学监护工作要点.方法:临床药师参与1例重症高龄患者的治疗,对营养支持、镇痛治疗、鼻饲药物的给予以及抗感染治疗等多方面提出具体意见,为患者制订个体化的给药方案,对护理人员及患者家属进行具体的给药指导.结果:通过药学监护,临床药师为临床提供了合理用药方案,取得了良好的治疗效果.结论:临床药师与医师、护士共同组成治疗团队,有利于提高临床治疗水平,促进合理用药.
