目的:评价某"三甲"医院糖皮质激素的应用情况.方法:采用销售金额、用药频度(DDDs)、药物利用指数(DUI)等指标,从该院2009年1-6月应用糖皮质激素的5 637份住院病历中,随机抽取507份进行统计、分析.结果:用药以治疗呼吸系统疾病、肿瘤/癌症、耳鼻喉疾病为主,中枢及周围神经系统疾病、自身免疫性疾病、肾胜疾病用药时间较长,用药量较大;抗炎应用最普遍,抑制免疫时用药剂量偏高;以地塞米松应用人数最多(50.49%),布地奈德销售金额最大(499.39元/人);有20例发生不良反应,6例禁忌证用药;有5例冲击疗法用药超过5d.结论:该院糖皮质激素以疗效显著、副作用小和价格便宜者应用最广,整体用药较合理;但应对糖皮质激素的冲击疗法、退热作用等加以重视.%OBJECTIVE: To evaluate the utilization of glucocorticoids in a grade three class A hospital. METHODS: 507 case histories, which were randomly collected from 5 637 cases treated with glucocorticoids in the hospital during Jan. ～Jun. in 2009,were analyzed statistically in respect of consumption sum, DDDs and DUI. RESULTS: Glucocorticoids were mainly used for patients with respiratory disease, tumor or cancer and ENT. The treatment for central and peripheral nervous system diseases, autoimmune diseases and kidney diseases lasted for a long time and adopted big dosage. Anti-inflammatory treatment was the most common and the dose of drugs for controlling immune was higher than normal level. The patients received dexamethasone were the most (50.49％) and the consumption sum of budesonide was the highest (499.39 yuan per capita). There were 20 cases of adverse drug reactions, and 6 cases received glucocorticoids within contraindication. The shock therapies of 5 patients were more than 5 days.CONCLUSION: Glucocorticoids are used popularly in our hospital, which have remarkable efficacy, less side effects and low prices. The utilization of it was rational on the whole. But we should pay more attention to shock therapy and fever-reducing effect of glucocorticoids.
