目的：探讨健康体检在早期2型糖尿病中的筛查作用，为临床提高2型糖尿病的早期检出率提供指导。方法200例进行血糖筛查的健康体检者作为研究对象，首先在体检中心进行健康体检，对其中空腹血糖水平高于正常值的体检者进行门诊综合检查，观察2型糖尿病确诊结果，并观察确诊为2型糖尿病患者中无典型糖尿病症状的例数。结果200例进行血糖筛查的健康体检者检查出98例空腹血糖高于正常值，检出率为49.0%，对其进行门诊综合检查，最终有90例患者确诊为2型糖尿病，检查准确率为91.8%(90/98)。90例确诊为2型糖尿病患者中，无糖尿病典型症状的患者占比为94.4%(85/90)，明显多于有糖尿病典型症状患者的5.6%(5/90)。结论健康体检在早期2型糖尿病患者中的有着良好的筛查作用，确诊率高，对于无典型性糖尿病症状的患者，可以大大减少漏诊的出现，便于早日使患者接受治疗，值得临床推广应用。%Objective To investigate screening effect by physical examination for incipient type 2 diabetes mellitus, and to provide guidance for clinical improvement of early detection rate in type 2 diabetes mellitus. Methods There were 200 people in physical examination for blood glucose screening as study subjects. They received physical examination firstly in examination center, followed by outpatient comprehensive examination for those with higher fasting blood glucose level than standard. Diagnosis outcomes of type 2 diabetes mellitus were observed, along with patients with atypical diabetic symptoms and diagnosed type 2 diabetes mellitus. Results There were 98 cases with detected higher fasting blood glucose level than standard among 200 people in physical examination, with detection rate as 49.0%. After outpatient comprehensive examination, there were 90 cases with diagnosed type 2 diabetes mellitus. Accuracy rate of examination was 91.8% (90/98). Among 90 cases with diagnosed type 2 diabetes mellitus, patients with atypical diabetic symptoms accounted for 94.4%(85/90), which was obviously higher than 5.6%(5/90) of cases with typical diabetic symptoms. Conclusion Physical examination contains excellent screening effect for incipient type 2 diabetes mellitus patients, along with high diagnosis rate. This method can considerably reduce misdiagnosed cases with atypical diabetic symptoms. It is helpful for early treatment in patients, and it is worth clinical promotion and application.
