目的 探讨癌症相关性乏力与鼻咽癌临床病理特征的关系.方法 回顾性分析214例鼻咽癌患者的临床资料,并依据Chalder乏力评价量表判断患者的乏力程度,分析乏力程度与临床病理参数的相关性.结果 214例鼻咽癌患者中有136例发生不同程度乏力,发生率为63.5％.乏力发生情况与年龄有关(P＜0.05),但与性别、肿瘤的分化程度均无关(P＞0.05).T1、T2、T3、T4期患者发生乏力的比例分别为52.9％、51.3％、72.5％和75.0％,随着T分期的增高,乏力发生的比例亦增高(P＜0.05).N1、N2、N3期患者发生乏力的比例分别为40.3％、75.4％和100.0％,随着N分期的增高,乏力发生的比例亦增高(P＜0.05).Ⅱ、Ⅲ、VA区淋巴结转移患者发生乏力的比例为65.7％,略高于Ⅰ、Ⅳ、VB、Ⅵ区淋巴结转移者的57.1％,但差异无统计学意义(P＞0.05).原发灶、颈部淋巴结放疗的疗效与乏力均无关.结论 鼻咽癌的癌症相关性乏力与性别、肿瘤分化程度、高低危区域淋巴结转移和放疗疗效无关,而与年龄、T分期、N分期有关.%Objective To determine the prevalence of cancer-related fatigue syndrome( CRFS) in a population of nasopharyn-geal neoplasms and investigate the relationship between CRFS and clinical variables. Methods The nasopharyngeal cancer patients were divided into two guoups:the fatigue group and the non-fatigue group,and their clinical data were collected and analyzed. Results One hundred and thirty-six patients(63. 5% ) were found to fulfill the criteria for CRFS. There was significant correlation between the CRFS and age ( P = 0. 000) , but with non-significat difference between the CRFS and sex ( P = 0. 482 ) , differentiation grade ( P = 0. 064). The rates of CRFS in T1, T2, T3 and T4 stages patients were 52. 9% ,51. 3% ,72. 5% and 75. 0% , while it was increasing with T stage( P <0. 05). The rates of CRFS in N,, N2, and N3 stages patients were 40. 3% , 75. 4% and 100. 0% , while it was increasing with N stage ( P < 0. 05 ). The rate of CRFS with II , M , V A areas lymph node metastasis was higher than that with Ⅰ , Ⅳ ,Ⅴ B, Ⅵ areas lymph node metastasis(65. 7% us. 57. 1% ) , however,the difference was not significant in statistics(P >0. 05). There was no correlation between the CRFS and the efficacy ( P > 0. 05 ) . Conclusion The CRFS of nasopharyngeal neoplasms relates to age,T stage and N stage. However, there is no relation among the sex,the areas of lymph node metastasis,differentiation grade,efficacy and CRFS.
