代谢综合征对冠状动脉血流速度的影响

摘要

Objective To observe coronary blood flow in patients with metabolic syndrome and explore the influence of metabolic syndrome on coronary blood flow in patients with normal coronary artery angiography. Methods We enrolled 42 patients with metabolic syndrome and normal epicardial coronary artery proven by coronary arteriography received in Dongying People′s Hospital from October 2011 to August 2013 as the metabolic syndrome group. And another 58 subjects without metabolic syndrome who also had normal coronary arteries proven by coronary arteriography in the same period were enrolled as the control group. The general data and the blood samples of the two groups were collected. Then we tested on the blood samples to determine total cholesterol ( TC ), low density lipoprotein cholesterin ( LDL-C ), triacylglycerol ( TG ), high density lipoprotein cholesterol ( HDL-C ), apolipoprotein A1 ( Apo A1 ), apolipoprotein B ( Apo B ), fibrinogen ( FIB ), fasting blood-glucose( FBG),blood urea nitrogen( BUN) and creatinine( Cr). By using the thrombolysis in myocardial infarction ( TIMI) frame count method,we measured the TIMI flow frames in the left anterior descending artery,left circumflex artery and right coronary artery of the two groups. Results The two groups were not significantly different(P>0. 05)in gender,age, TC,LDL-C,Apo A1,Apo B,FIB,FBG,BUN and Cr. The metabolic syndrome group was higher(P <0. 05)in the prevalence of hypertension,prevalence of diabetes mellitus,smoking or not,alcoholic consumption,BMI,TG,SBP and DBP and was lower(P<0. 05)in HDL-C than the control group. The metabolic syndrome group was higher(P<0. 05)than the control group in the average TIMI flow frames in the left anterior descending artery,left circumflex artery,right coronary artery and coronary artery. Multivariate regression analyses showed that BMI,DBP and TG were the influencing factors(P<0. 05) for coronary blood flow velocity. Conclusion The coronary blood flow is relatively slow in patients with metabolic syndrome. BMI,DBP and TG are influencing factors for coronary flow velocity.%目的:观察代谢综合征( MS)患者冠状动脉血流情况,探讨MS对冠状动脉造影正常者血流速度的影响。方法选取2011年10月—2013年8月在东营市人民医院行冠状动脉造影显示心外膜冠状动脉无明显病变的MS患者42例,为MS组;另选取同期因心脏症状行冠状动脉造影无明显病变,且不符合MS者58例作为对照组。收集两组一般资料,并采集血液,检测总胆固醇( TC)、低密度脂蛋白胆固醇( LDL-C)、三酰甘油( TG)、高密度脂蛋白胆固醇(HDL-C)、载脂蛋白A1(Apo A1)、载脂蛋白B(Apo B)、纤维蛋白原(FIB)、空腹血糖、尿素氮(BUN)、肌酐(Cr)等指标。采用心肌梗死溶栓试验(TIMI)血流计帧法测量两组左前降支、左回旋支及右冠状动脉血流速度。结果两组性别、年龄、TC、LDL-C、Apo A1、Apo B、FIB、空腹血糖、BUN、Cr比较,差异均无统计学意义(P>0.05);MS组高血压患病率、糖尿病患病率、吸烟率、饮酒率、BMI、TG、SBP、DBP均高于对照组,HDL-C低于对照组(P<0.05)。MS组左前降支、左回旋支、右冠状动脉、冠状动脉平均TIMI血流帧数均高于对照组(P<0.05)。多元线性回归分析结果显示,BMI、DBP、TG是影响冠状动脉血流速度的因素( P<0.05)。结论 MS患者冠状动脉血流速度减慢,而BMI、DBP及TG是影响冠状动脉血流速度的因素。
