血小板参数与脑梗死患者颈动脉狭窄的相关性研究

摘要

目的 探讨血小板参数即血小板计数(PLT)、血小板平均体积(MPV)、血小板分布宽度(PDW)和血小板压积(PCT)与脑梗死患者颈动脉狭窄的相关性.方法 选取2011年1月-2012年6月在我院住院治疗的急性脑梗死患者295例(脑梗死组),依据颈动脉超声检查结果有无颈动脉颅外段狭窄及其程度分为:无狭窄组(155例)、轻度狭窄组(68例)、中度狭窄组(41例)和重度狭窄组(31例).另选取同期体检健康者96例(对照组).应用全自动血细胞分析仪检测受检者PLT、MPV、PDW和PCT水平,并进行比较分析.结果 脑梗死组PLT、MPV、PDW水平与对照组比较,差异均有统计学意义(P<0.05).脑梗死患者不同颈动脉狭窄组PLT、PDW、PCT水平比较,差异均无统计学意义(P>0.05);而MPV水平比较差异有统计学意义(P<0.05),其中轻、中、重度狭窄组MPV水平与无狭窄组比较,重度狭窄组MPV水平与轻、中度狭窄组比较,差异均有统计学意义(P<0.05).Pearson相关分析显示MPV与颈动脉狭窄程度呈正相关(r=0.408,P<0.01).结论 MPV增高与脑梗死患者颈动脉狭窄及其程度密切相关.%Objective To investigate the relationship of platelet count ( PLT ), mean platelet volume ( MPV ), platelet distribution width ( PDW ) and plateleterit ( PCT ) with carotid artery stenosis in acute cerebral infarction patients. Methods A total of 295 patients with acute cerebral infraction ( cerebral infarction group ) admitted to our hospital from January 2011 to June 2012 were divided into non - stenosis group (155 cases ), slight stenosis group ( 68 cases ), moderate stenosis group ( 41 cases ) and severe stenosis group (31 cases ) based on the severity of carotid artery stenosis. Another 96 healthy people during the same period were involved into the study as control group. The PLT, MPV, PDW and PCT values were detected by automated blood cell analyzer and were compared between the two groups. Results Compared with the control group, the PLT, MPV and PDW levels in cerebral infarction group showed statistically significant differences ( P < 0. 05 ) . The levels of PLT, PDW and PCT in different carotid artery atenosis groups showed no statistically significant difference ( P > 0. 05 ), but there was statistically significant difference in MPV levels ( P < 0. 05 ) . The levels of MPV in the slight, moderate and severe carotid stenosis groups showed statistically significant differences compared with that of the non - stenosis group, and the levels of MPV in the severe carotid stenosis group showed statistically significant differences compared with that of the slight and moderate carotid stenosis group ( P < 0. 05 ) . Pearson correlation analysis showed that the MPV level was positively correlated with the degree of carotid artery stenosis ( r =0. 408 , P <0. 01 ) . Conclusion Increased MPV is closely associated with carotid artery stenosis and its degree in cerebral infarction patients.
