Objective To investigate the risk factors for incision mycoplasma infection after cesarean section. Methods A total of 21 cases of incision infections happened among 2 570 cases of cesarean section in our hospital between January 2009 and December 2011. The 21 cases were assigned into the study group, and 87 cases without postoperative incision infections and assigned into the control group. Mycoplasma test kit was used for mycoplasma cultivation and detection. Age, hemoglobin, body weight, parity, days of pregnancy, premature rupture of fetal membranes, vaginal examination times, characteristics of amniot-ic fluid, and duration of operation were assessed using unconditional logistic regression analysis. Results The incision secretion culture results showed 17 positive and 4 negative cases. Mycoplasma was cultured in 15 cases. Ciprofloxacin resistance rate was 13/17, Josamycin and doxycycline susceptibility rates were 15/16 and 16/16, respectively. Contaminative degree of amniotic fluid and operation duration were the risk factors for incision infection after cesarean sections ( P < 0. 05 ) . Conclusion To prevent incision infection after cesarean section, the duration of operation should be shortened. Preoperative screening for the genital tract mycoplasma should be carried out regularly. Josamycin and doxycycline should be administered orally 6 hours postoperatively in addition to the routine use of cephalosporins for pregnant women positive for genital tract mycoplasma check.%目的 探讨剖宫产术后切口支原体感染的危险因素,为临床预防提供依据.方法 选取我院2009年1月-2011年12月剖宫产手术病例资料2 570例,发生术后切口感染21例.支原体培养采用珠海迪尔生物有限公司的支原体一体化试剂盒,严格按照试剂盒要求进行培养鉴定.21例切口感染的病例作为感染组,随机选择同期术后无切口感染的87例作为对照组,对可能影响切口感染的因素:年龄、血红蛋白、体质量、产次、妊娠天数、胎膜早破、阴检次数、羊水性状、手术时间进行非条件Logistic回归分析.结果 切口分泌物培养结果:17例阳性,4例阴性,其中15例培养出支原体.支原体药敏结果:环丙沙星耐药率高达13/17,交沙霉素和多西环素的敏感率分别为15/16和16/16.羊水性状和手术时间是剖宫产术后切口感染的危险因素(P＜0.05).结论 预防剖宫产术后切口感染应当缩短手术时间,重视生殖道支原体感染.术前应当常规开展生殖道支原体筛查,对生殖道支原体阳性的产妇,除常规应用头孢类抗生素以外,术后6 h口服交沙霉素或多西环素治疗.
