Objective To investigate the outcome of traumatic chronic tibial osteomyelitis after stuffing operation with musculocutaneous flap or fasciocutaneous flap combined with wound irrigation. Methods A total of 26 patients with traumatic chronic tibial osteomyelitis treated hy our hospital from February 2008 to October 2010 were enrolled. During the operation , bigger bone cavities with severe infections were stuffed with muscle or musculocutaneous flaps while smaller bone cavities with slight infections were stuffed with fasciocutaneous or fasciosubcutaneous flaps after thorough surgical debridement of the wounds. Postoperative irrigations lasted for 7 ～ 10 days. Results All the wounds healed by first intention. A follow - up of 3 ～ 36 months disclosed no recurrence. Conclusion The combination of thorough debridment, stuffing operation with musculocutaneous or fasciocutaneous flaps and continuous irrigation is reliable and effective for the treatment of traumatic chronic tihial osteomyelitis.%目的 探讨手术加创面灌洗治疗创伤后慢性胫骨骨髓炎的疗效.方法 选取2008年2月-2010年10月收治的创伤后胫骨慢性骨髓炎患者26例.彻底清创后,对骨腔大、感染重者用肌瓣或肌皮瓣填塞;骨腔小、感染轻者用筋膜皮瓣加筋膜皮下组织瓣修复,术后创面持续灌洗治疗7～10 d.结果 26例患者术后创面全部一期愈合,随访3～36个月,无一例复发.结论 彻底清创,应用肌皮瓣和筋膜皮瓣填塞修复创面,术后持续灌洗治疗可以有效治疗创伤后慢性胫骨骨髓炎.
