Purpose To observe the different expression of BCL-2, CK20, CD10, androgen receptor ( AR ) in basal cell carcinoma ( BCC ) and trichoblastoma ( TB ), and to improve diagnosis and differential diagnosis.Methods 32 cases of hasal cell carcinoma and 8 cases of trichoblastoma were studied by using of immunohistochemical staining for BCL-2, CK20 , CD10 and AR.The expression of BCL-2 , CK20 , CD1O and AR in tumor cells and surrounding stromal were observed.Results 30 cases of BCC were BCL-2 diffusely positive in the tumor cells ; CK20 were negative in all cases; CD10 expression was seen in some tumor cells in 23 cases and negative expression in 9 cases, but 4 cases of tumor stroma were positive for CD10; AR expressed in 7 cases.8 cases of TB were partly positive for BCL-2.CK20 was scattered positive in 6 cases and negative in 2 cases.CD10 was positive in some tumor cells in 4 cases and negative in 4 cases.3 cases of interstitial were positive and negative in 4 cases, AR was negative.BCL-2 and CK20 expression in the two group had statistically significant difference ( P ＜ 0.01 ), AR and CD10 was no significantly difference.Conclusion BCL-2 diffuse strongly positive.CK20-negative support the diagnosis BCC, otherwise support the diagnosis of TB.%目的 观察BCL-2、CK20、CD10、AR在基底细胞癌(basal cell carcinoma,BCC)和毛母细胞瘤(trichoblastoma,TB)中表达的不同,以提高它们在两者诊断和鉴别诊断中的作用.方法 对确诊的32例BCC和8例TB石蜡包埋标本行BCL-2、CK20、CD10、AR免疫组化染色,观察它们在肿瘤细胞及其周围间质中的表达.结果 32例BCC中BCL-2肿瘤上皮弥漫阳性表达30例,阴性2例;CK20均为阴性;CD10肿瘤上皮部分表达23例,阴性9例,肿瘤间质阳性4例,阴性28例;AR阳性表达7例.8例TB中BCL-2上皮部分均阳性表达,CK20散在阳性6例,阴性2例,CD10部分上皮阳性4例,阴性4例,间质阳性3例,阴性5例;AR均为阴性.统计结果,BCL-2、CK20表达差异有显著性意义(P＜0.01),AR、CD10差异无显著性意义.结论 BCL-2弥漫强阳性、CK20阴性支持诊断BCC,反之支持诊断TB.
