目的 观察超声内镜指导下套扎器套扎治疗胃间质瘤的效果.方法 选取经超声内镜检查诊断胃黏膜下来源于胃固有肌层的小间质瘤45例,瘤体直径0.5 ～1.3 cm,以六连环套扎器行内镜下间质瘤套扎治疗,套扎后用超声内镜观察间质瘤是否套入圈套中.术后观察有无出血、穿孔等并发症.术后1～3月复查胃镜以了解愈合情况.结果 胃黏膜下来源于固有肌层小间质瘤45例中,位于胃底32例,胃体11例,胃窦2例.7例行细针穿刺组织病理及免疫组化结果证实为黏膜下间质瘤.行胃黏膜下间质瘤的套扎术,成功44例,成功率97.8％.3例术后黑便,无一例发生穿孔.结论 在超声内镜指导下对胃黏膜下固有肌层小间质瘤行套扎术,方法简便、安全、有效,并发症少,值得临床广泛应用.%Objective To observe the effect of ligation in gastric submucosal stromal tumors originated from the muscu-laris propria layer directed by endoscopic ultrasonography ( EUS). Methods 45 patients with gastric submucosal stromal tumors( GISTs) originated from the muscularis propria layer were treated with ligation guided by EUS. Thereafter, all patients were followed up with routine endoscopy after 1 ~ 3 months. The diameters of tumors were from 0. 5 to 1. 3 cm. Results Among 45 patients with small GISTs, there were 32 cases of fundus, 11 cases of corpus, 2 cases of antrum. 7 cases were undergone fine needle biopsy and coincidence rate of pathology was 100% . The successful rate of endoscopic band ligation was 97. 8% . 3 cases suffered from melena. No perforation, massive hemorrhage, or other complication requiring endoscopic or surgical intervention occurred. Conclusion This method was simple, safe and effective. It should be popularized.
