Objective: To investigate the feasibility and advantages of image-guided endoscopic transnasal odontoidectomy to treat cervicomedullary compression with basilar invagination.Methods: A male patient with basilar invagination underwent image-guided endoscopic transnasal odontoidectomy.Results: A successful resection of odontoid process was achieved.Remarkable neurological recovery was observed after surgery in this patient without obvious sugical-related adverse effect.Conclusions: As a new approach for odontoideclomy, endoscopic transnasal odontoidectomy has advantages such as improved visualization, avoiding palatal incision and nasogastric feeding over the traditional approach.%目的:探讨导航内镜下经鼻齿状突切除治疗颅底凹陷畸形的可行性和优缺点.方法:在导航内镜下经鼻入路切除1例患者的齿状突.结果:齿状突切除满意,患者症状和体征改善明显,无手术相关并发症发生.结论:在导航内镜下经鼻切除齿状突作为一种齿状突切除新的方式较传统手术方式有改善视野、避免切开软腭和术后鼻饲等诸多优势.
