心脏骤停猪复苏期间动静脉血气的分析

         

摘要

目的 比较心脏骤停猪复苏中动、静脉血气的变化,以了解动静脉血气结果 对判断复苏效果的意义.方法 24头家猪电刺激制作心脏骤停心室纤颤(VF)模型,VF后9 min后进行CPR,并分别抽取复苏前后动脉(CA)和混合静脉(MV)血进行分析.结果 VF期间PMVO2较VF 前明显下降(P<0.05),而PMVCO2明显升高(P<0.01).中心动脉及外周动脉血气均示pH值及乳酸(Lac)升高,而HCO-3值较正常均有降低;混合静脉血示pH值降低,而Lac逐渐升高.结论 复苏期间存在氧供和酸碱失衡,分析静脉血气对了解机体组织的氧合和酸碱状态更切合临床实际.%Objective To invesligale ihe changes of blood gases of artery and venous in cardiac arresl pigs during cardiopulmonary resuscilalion ( CPR). To make ihe significance of blood gases analysis in artery and venous lhal eslimales ihe effective of CPR. Methods 24 domestic pigs, cardiac arresl (CA) venlricular fibrillalion (VF) was induced by electrical slimulalion inslrumenls. Afler 9 min VF, ihe slandard CPR was done. The blood samples were drawn from cenlral artery (CA) and mixed venous (MV) blood during CPR. Results In VF 9 min, PMV 02 decreased which compared wilh VF before (P<0. 05) , however, PMVCO2 showed an obvious increase (P<0. 01). PH and Laclale value were increased in all ihe blood gas analysis of CA and PA, bul HCO3 - decreased. PH value was decreased in MV, bul Laclale had gradually increased afler CPR. Conclusion There were oxygen delivery and acid base imbalance during ihe CPR. The blood gas analysis of mixed venous was more corresponded wilh clinical realily lo realize ihe organizalion oxygenalion and acid - base slalus.
